What are we trying to help, and how will we know?
Medication decisions consider symptoms, treatment history, physical health, interactions, preferences, and the responsibilities of daily life.

An explanation you can follow.
We discuss intended benefits, possible adverse effects, alternatives, and monitoring. Recommendations may involve starting treatment, changing a dose, simplifying a regimen, or continuing an effective approach.
Review what changes.
Follow-up considers benefit, side effects, functioning, medication use, and safety. Relevant laboratory testing or coordination with primary care may be recommended. Your observations help guide adjustment.
Medication within the whole picture.
Sleep, physical health, therapeutic work, relationships, and substance use can influence outcomes. Prescribing follows clinical assessment and applicable requirements; each medication choice depends on suitability.
Established patients use CharmHealth for medication questions and refill requests. Urgent medical or psychiatric concerns require immediate assessment through appropriate emergency services.
Monitoring follows the medication and the person.
Before prescribing, we review relevant health conditions, current medications, supplements, allergies, past responses, and potential interactions. Depending on the treatment, follow-up may include symptoms, blood pressure, weight, laboratory testing, or other safety measures.
Ask how to take a medication, what adverse effects to report, and how to reach the practice with concerns. Dose changes and discontinuation should be planned with the prescriber.

The conversation behind a prescription.
Medication decisions involve more than matching a symptom with a drug. I consider the working diagnosis, past treatment, physical health, other medicines and supplements, daily routines, and your preferences. A useful recommendation has a clear purpose and a plan for review.
Discuss what change to look for, how benefit will be assessed, which adverse effects need attention, and when to follow up. The monitoring plan depends on the medicine and your circumstances. A follow-up visit reviews your experience alongside relevant clinical information.
If an approach is difficult to tolerate or has limited benefit, the next step is to reassess. Diagnosis, dose, duration, interactions, consistency of use, and other conditions can affect the picture. Changes are made through shared discussion and clinical judgment; improvement follows an individual course.

Before you begin.
Practical questions about medication management.
Can you review medication prescribed elsewhere?
Bring the medication name, dose, schedule, reason it was prescribed, and the prescriber’s details. Relevant records can help. Assuming prescribing responsibility requires assessment, clinical suitability, and a clear plan; appointment requests alone leave that transfer unconfirmed.
What if I am concerned about side effects?
Contact the prescribing clinician promptly about new or concerning symptoms and ask for guidance before changing the dose or stopping. Bring a description of when the change began and how it affects you. A severe reaction or immediate danger requires emergency care.
What if I would prefer to avoid medication?
Your preferences belong in the assessment. Discuss what you hope to improve, previous experiences, and the tradeoffs you want to understand. Therapy, medication, combined care, or another approach can be considered according to the clinical findings.
How do refills and ongoing follow-up work?
Review the practice’s refill process during your visit and use the secure portal for clinical requests. Follow-up, safety monitoring, and prescribing requirements affect refills. Ask about the process before your supply runs low, particularly around travel or schedule changes.
